1. What is a home health care?
           A home health care is a health service provided in the patient's home by a health care
    professional (Registered Nurse, Licensed Pratical Nurse, Physical Therapist,
    Occupational Therapist, Speech Therapist, Home Health Aide and/or Medical Social
    Worker).

2. How would I know if I am qualified for home health care services?
          a. If you are homebound due to illness or disability.
          b. If your physician authorizes you for a home health care services.

3. How much does it cost for me to get home health care services?
           Home health care services are usually paid by Medicare, Medicaid, HMO and/or private
    pay. Different rates to different services provided. Do not hesitate to ask our office about it.

4. What should I do to start receiving home health care services from you?
            Give our office a call and let us know that you or someone from your family is interested
    in receiving our services. Our company also accepts referrals from friends, social workers,
    or doctors.

5. Could I transfer to Gold Health Home Care, if I am receiving care from a different
     company?
           It is your right to choose which company you would like to receive home health services
     from. Just let your present home health care company know that you want our services
     then our company can start serving you.
